Whitepaper Finds Digital Value-Based Care Reduces Readmission Rates of Patients With Congestive Heart Failure

July 30, 2021

Veta Health sponsored research into how digital disease management tools could reduce hospital readmissions for patients with Congestive Heart Failure (CHF). Researchers noted that upon adoption of these tools, a healthcare facility noted a 78% decrease in hospital readmissions.

“Overall, the findings suggest that the integration of a digital disease management platform holds great potential for management of CHF and other chronic conditions, particularly as the proportion of elderly Americans diagnosed with chronic conditions continues to increase.” Download the whitepaper here.
